5. Legible photocopies of Computerized National Identity Cards of all founding members and two witnesses.
6. Treasury challan for Rs. 1500 under the Head of Account C03545-Registration of Societies. Should be deposited in State Bank of Pakistan or any branch of National Bank of Pakistan.
